PASSPORT Information


PASSPORT (Pre-Admission Screening System Providing Options and Resources Today) provides
families alternatives to traditional nursing home placement PASSPORT provides essential in-home
services and can curb the skyrocketing costs of nursing home care in Ohio. This long-term care
program is funded by the Ohio Department of Aging and the Ohio Department of Job and Family
Services.

AAA9 is the PASSPORT program administrator responsible for participant assessment,
case management and contract management with service providers in our nine county radius.Your case manager will continually assess your needs throughout your enrollment. On-going assessment, coordination and monitoring of a consumer's needs, strengths and circumstances and services to assure that services/interventions continue to be appropriate for your care.

“Case management" means coordinating and monitoring the delivery of all services identified on the
service plan; periodic re-evaluation of the consumer's goals and objectives for long-term care
services; periodic re-determination of program eligibility; authorization of the amount, scope,
and duration of long-term care services; and assisting the consumer to access needed medicaid-waiver
services and other medical and social services, regardless of their source of funding.
