RN or LSW Case Managers

The Area Agency on Aging Region 9, Inc.
(AAA9), located in Cambridge OH, is interviewing for Case Managers. Applicants
must be either a Licensed Social Worker (LSW) or a Registered Nurse
(RN)
 with one year of experience working with individuals with
physical or behavioral disabilities; in home health care, medical social work;

Assessor/Long Term Care Specialist   

General Duties:

Provides assessment, information, and linkage for long-term care applicants, participate in community collaboration. Conduct assessments for PASSPORT, Assisted Living Waiver, NH Level of Cares, Long Term Care Consultations, Home Choice, Residential State Supplement LOC, Community Living Care transitions assessments, and all associated follow up related to assessment processes as defined.

Preferred Experience/Skills:

  1. Five years’ experience in home health care, medical social work, or geriatrics.
  2. Registered Nurse (RN) or Licensed Social Worker License (LSW) required
  3. Successful completion of ODA PP certification. 
  4. Knowledge of AAA9 programs and community resources
  5. Excellent computer and communication skills
  6. Strong clinical judgement required.

 

ASSESSOR Homecare Waiver (Full Time)

Remote-Based Position

 General Duties:

Provides assessment, information, and linkage for long-term care applicants in 14 county coverage area, participate in community collaboration. Conduct assessments for OHCW and all associated follow up related to assessment processes as defined. May serve as back-up to Case Management.

Rotates On-Call coverage with colleagues and other PAAs during nights, weekends and holidays to ensure consistent and appropriate after-hours coverage.

Required Experience/Skills:

  1. Registered Nurse (RN)
  2. One year of experience working with people with physical or behavioral disabilities; in home health care, medical social work; or behavioral health
  3. Must have a reliable car or means of transportation, a valid driver’s license and proof of automobile
  4. Must have high speed internet at residence and cell phone

Position Location:

The OHCW Case Manager position is remote based. **Please note, initial and on-going training, individual performance and other factors may require office days based on unit/division/agency needs.

 

Case Assistant – Clinical Quality & Compliance Unit (Full Time) Office-Based Position

General Duties:      

Provide client and staff related clerical support to Clinical Quality and Compliance Unit. Generate, copy, process, log, track and follow-up when needed; Email/Fax documents. Entering documentation into PIMS system; Assist with statistical record keeping, creation and revision of unit policies and reports; Back-up to PP/ALW/Assessment Case Assistants; Additional responsibilities as assigned

 

Required Education & Experience:

  1. High School Diploma
  2. Possess at least one year experience in the following:

Providing individuals with information about options available to meet long-term care needs or providing individuals with related information such as that which is provided through information and referral, information and assistance; or Managed care experience or other such health care experience related to the long-term care needs of the individual.

  1. Excellent telephone and communication skills (verbal and written).
  2. Must have excellent computer skills and be able to effectively use Microsoft Office software including Word, Excel & Email
  3. Experience in a medical or Medicaid/Medicare facility preferred

 

Clinical Quality and Compliance Specialist (Full Time)

Remote-Based Position                                  

General Duties:      

Develop and implement quality and compliance functions within the Passport/ALW Unit (Assessment & CM).  These functions include but are not limited to the quality/compliance areas as follows:

  • Gather, evaluate, and analyze data to promote quality practice standards
  • Conduct clinically based audits
  • Conduct 1:1 conferences with clinical staff
  • Lead efforts related to quality & compliance with relevant rules, regulations and internal practices/policies
  • Incident Management Specialist Back-Up
  • Create, review, revise, update policies and procedures
  • Provide guidance and develop training on quality/compliance focus areas
  • Maintain statistical record keeping.
  • Monitor/track trends and provide feedback and make recommendations for operational improvements
  • Assist with new and tenured staff training
  • Assist with management reports

Required Experience/Skills:

  1. Registered Nurse (RN) or Licensed Social Worker (LSW).

Specialty Case Manager (Full Time) Remote-Based Position

Duties:

Position will serve as a Specialty CM for the Passport/ALW Case Management unit.  Duties include but are not limited to the specialty areas as follows:

  • Cover/CM select groups of individuals such as:
    • Newly enrolled consumers
    • Consumers enrolled from the NF
    • Consumers without full Medicaid
    • High risk consumers due to health and safety issues
  • Assist with coverage contacts and visits
  • Assist with chart reviews
  • Monitor/track trends related to the specialty groups of consumers and focus areas
  • Assist with new and tenured staff training
  • Assist with management reports

SRS Recovery Manager– Licensed or Non-Licensed Professional (Full Time) Remote-Based Position

 Caseload in the 14-county area that is served by this program

 General Duties:      
Recovery Manager of consumers in the Specialized Recovery Services program. Outreach and Assistance to individuals to ensure follow-up with behavioral health providers or their specialty physicians. Coordination of ongoing care. Linkage and referrals. Assistance with managing ongoing Medicaid. Required contacts, visits, and eligibility determinations. On-call participation required.

 

Required Experience/Skills:

  • Must be a Registered Nurse, Licensed Social Worker or have a Bachelor’s degree in a related field.
  • Minimum of three years post degree mental health experience and/or one year experience with Individual’s with diagnosed chronic conditions.
  • Must have a reliable car or means of transportation, a valid driver’s license, and proof of automobile insurance.
  • Must have high speed internet at residence and cell phone coverage.

Long-Term Service & Support Intake Specialist (Full-Time)   

Office-Based Position

 General Duties:    

Will assist individuals over the phone and in person with identifying, planning for, and securing long term care programs and services through the following: complete long-term service and support questionnaires, provide individualized support navigation, assist with information & referral, provide supportive follow up, schedule ODA assessments, and work with community partners for growth and maintenance of resource information and availabilities.

Required Experience/Skills:

High school diploma required; Additional education preferred.

Three years of experience in a high-volume medical or long-term care setting serving aged or disabled populations preferred.

Technology fluency required.

Customer service excellence required.
