Specialist

Area Agency on Aging, Region 9 – Cambridge, Ohio

The Area Agency on Aging, Region 9 is seeking a compassionate and motivated individual to join our team as an Assessor/Long-Term Care Specialist. While the position offers remote work flexibility, it requires travel throughout our service area, conducting assessments, as well as occasional in-office meetings, trainings, and team events. This position would require you to reside within reasonable commuting distance of our office and service area. We cover Belmont, Carroll, Coshocton, Guernsey, Harrison, Holmes, Jefferson, Muskingum, and Tuscarawas Counties in Ohio. 

Provides assessments, information, and linkage for long-term care applicant. Conduct in-person assessments for PASSPORT Waiver, Assisted Living Waiver, MyCare Waiver, Care Coordination, Home Choice, and CLS Programs. 

In this role, you’ll help older adults and individuals with disabilities remain safe, independent, and supported in their own homes while building a rewarding career with work-life balance.

Required Experience/Skills:

• Five years’ experience in health/human services or home care agency administration required. 
• Registered Nurse (RN), or Licensed Social Worker (LSW), or LISW preferred
• Supervisory or management and computer experience required
