PASSPORT Case Manager/Waiver Service Coordinator

The Are Agency on Aging, Region 9 is seeking a compassionate and motivated individual to join our team as a PASSPORT Case Manager/MyCare Waiver Service Coordinator. We cover a nine-county region including Belmont, Carrol, Coshocton, Guernsey, Harrison, Holmes, Jefferson, Muskingum, and Tuscarawas Counties in Ohio. While the position offers remote work flexibility, it requires travel throughout our service area, including in-home visits with consumers, as well as occasional in-office meetings, trainings, and team events. This position would require you to reside within a reasonable commuting distance from our office and service area.
- Active LPN
- Active RN
- Active LSW
- Non-Licensed: Must possess a bachelor’s degree in social work, counseling, gerontology, psychology, or similar field and a minimum of one year experience with home and community-based services.
In this role, you’ll help older adults and individuals with disabilities remain safe, independent, and supported in their own homes while building a rewarding career with work-life balance.
