Long Term Care ConsultationLong Term Care Consultations (LTCC) provide individuals or their representatives with information about options available to meet their long-term care needs and factors to consider when making long-term care decisions. Particular emphasis is on the explortation of whether there are appropriate community service alternatives to nursing facility placement that are a viable and feasible option to meet the individuals needs and preferences. Anyone may request a consultation regardless of whether or not they are required to receive it.

The mission of the Long Term Care Consultation Program is to:

Empower each individual to make informed decisions about the best setting to meet their needs.

Provide information about the options available to meet all your long-term needs and what factors to consider when making long-term care decisions.

Explore all service alternatives, regardless of a specific service setting being sought by the individual.

 

Who is Eligible for a Long Term Care Consultation?

Everyone is eligible for a Long Term Care Consultation. Whether you are a caregiver for a loved one, are concerned about your own long-term care needs, or just planning for your future, a Long Term Care Consultation may help you decide what is best for you. The consultation is free and there are no financial considerations.

A Registered Nurse (RN) or a Licensed Social Worker (LSW) will come to your home and perform an assessment of your needs. At the end of the assessment they will provide you with:

Long-term care options available to the consumer (private and public) that may meet your needs.

Information explaining the methods to use to apply for long-term care options.

A personalized list of factors to consider when deciding on long-term care options mentioned above.

Information explaining the opportunities and methods for maximizing the independence and self-reliance of the consumer. This will include information on support services, family, friends, and community.
