Assisted Living Program Information

Assisted LivingOhio's Assisted Living Waiver Program provides services in certified residential care facilities to help delay or prevent nursing facility placement. Assisted Living promotes aging in place by supporting the Consumers desire for independence, choice, and privacy and is less expensive and less restrictive..

Mother and DaughterYou may be eligible if?
Eligible participants must be current nursing facility residents or existing Medicaid waiver (PASSPORT, Ohio Home Care, Choices) participants, be age 21 or older, needs hands on assistance with dressing, bathing, toileting, grooming, eating or mobility and have unpredictable needs for assistance with activities of daily living. There are also financial criteria for Medicaid eligibility and they must be able to pay room and board.
